Returning Patients:
Please arrive 10 minutes before your scheduled appointment. Be sure to bring your current
insurance card and notify the front desk of any insurance, phone number or address changes.
Copay:
If your insurance requires a copay, this will be collected when you sign in with the receptionist.
Late Policy:
In order to minimize the waiting time of our patients, we reserve the right to reschedule your
appointment if you are more than 10 minutes late.
Cancellation Policy:
If you are unable to keep your appointment, please call our office 24 hours in advance to
reschedule. If you fail to contact us prior to your appointment, you will be subject to a $55.00
fee. If you miss two appointments without providing prior notification, you will be subject to discharge
from the practice.
Prescriptions:
Our office will have access to your medication profile after your initial visit. Please let your
physician know of any medication refills that you may need at the time of your appointment.
If you need a prescription refilled prior to your appointment, please call our office with the
following information:
• Medication name
• Dose (amount you are taking)
• Frequency (how often you are taking it)
• Name, location, and phone number of your pharmacy
A prescription refill request will require 24-48 hours to process.
Hospitals:
Dr. Winters provides inpatient care at Fort Hamilton Hospital in Hamilton. If you choose
another facility for inpatient care, we will obtain your records to ensure the best possible follow up care.
Diagnostic Testing:
It is important for you to verify with your insurance plan their preferred location for laboratory
testing and imaging services. We provide laboratory services in the office through LabCorp.
Fort Hamilton Hospital is a convenient location to obtain diagnostic tests for patients without
insurance, as they offer discounted rates for most tests.We usually receive test results within 1-2 business
days. You will be contacted with your results by phone or via our patient portal.
What is a D.O.?
D.O. stands for doctor of osteopathic medicine. This credential means that the physician has completed
all of the traditional education and training to provide medical care including medical school and
residencies. Further, D.O.s receive additional emphasis on the musculoskeletal system (bones and
muscles in the body) and incorporate osteopathic manipulation treatment into their practices.
Osteopathic manipulation, an emphasis on disease and injury prevention, and a holistic treatment
approach lead to integrated patient care and provide the foundation for care provided by the D.O.
